  • Input required: Enter the home phone number for the patient.

10. Work Phone

  • Input required: Enter the work phone number for the patient.

11. Wireless Phone

  • Input required: Enter the wireless phone number for the patient.

12 . Preferred Confirm Method

  • Input required: Specify the preferred method for confirming appointments.

13 . Birth Date ( YYYY-MM-DD)

  • Input Required: Enter the birth date of the patient in the specified format.
